OCT Scan vs Retinal Photography: The Key Difference

It records features including the optic nerve, blood vessels and central retina, or macula.

An OCT scan, short for optical coherence tomography, works differently. It uses light to produce cross-sectional images of the retina and optic nerve. Rather than looking only at the surface, it allows your optometrist to view the retina in layers – rather like seeing a slice through a cake rather than just its icing.

Neither test replaces a thorough eye examination. Your optometrist will still assess your vision, discuss any symptoms, examine the front and back of your eyes and consider your wider health. Imaging adds another valuable level of detail to that clinical assessment.

What Retinal Photography Can Show

A retinal photograph provides a lasting visual record of the back of your eye. This is especially valuable because subtle changes can be difficult to judge from memory alone. Comparing an image from this year with one taken at a previous appointment may help identify changes that deserve closer attention.

Your optometrist may use retinal photography to monitor the appearance of the optic nerve, which is particularly relevant when assessing glaucoma risk. Around the optic nerve, it can measure the retinal nerve fibre layer, providing useful information when investigating or monitoring glaucoma.

This does not mean an OCT scan diagnoses every eye condition by itself. Scan results need careful interpretation alongside eye pressure readings, visual field testing, retinal appearance, symptoms and risk factors. A scan can occasionally show a variation that is normal for that individual, while a normal-looking scan does not remove the need to investigate concerning symptoms.

Like retinal photography, OCT is painless and does not touch the eye. Most scans take only a few moments. You look at a target inside the instrument while it captures a series of images. The technology is sophisticated, but the experience for the patient is straightforward.

Why OCT is useful for monitoring

One of OCT’s greatest strengths is repeatability. If you have an area that needs observation, scans taken over time can help show whether the retinal layers are stable or changing. That can support more confident decision-making, whether the right next step is continued monitoring, a shorter recall interval or an ophthalmology referral.

This is particularly valuable for people with a family history of glaucoma, raised eye pressure, diabetes, macular changes, high short-sightedness or previous findings that need watching. It may also be recommended if you notice blurred central vision, straight lines appearing wavy, a new patch of missing vision or unexplained changes in sight.

Is an OCT Scan Better Than Retinal Photography?

It depends entirely on the question being asked. OCT offers greater depth and detail of specific retinal and optic nerve structures. Retinal photography gives a wider colour overview and a highly useful record of the visible back of the eye.

If your optometrist is checking the overall appearance of your retina and blood vessels, a retinal photograph may be the most appropriate imaging tool. If there is concern about the layers of the macula, fluid beneath the retina or possible glaucoma-related nerve fibre loss, OCT can provide information that a photograph cannot.

Often, the tests complement one another. A photograph may show a feature that needs closer analysis with OCT. Equally, an OCT scan can be interpreted more clearly when viewed alongside a colour retinal image and the findings from a full examination.

It is also worth remembering that the right test is not always the newest or most detailed one. Good eye care means choosing the investigation that is clinically relevant for you, explaining what it can and cannot tell us, and arranging appropriate follow-up when needed.

Who May Benefit From Both Tests?

Imaging may be particularly useful during a comprehensive eye examination if you are over 40, have a family history of glaucoma or macular disease, live with diabetes, take medication that can affect the eyes, or have noticed a change in your vision. It can also be helpful for people with high prescriptions, especially high myopia, because this can be associated with changes at the back of the eye.

For children and younger adults, imaging may be recommended when there is a specific clinical reason, rather than as a one-size-fits-all extra. A tailored approach matters. The best plan for a child having their first eye examination will not necessarily be the best plan for an adult monitoring long-term eye health.

Professionals who spend long periods on screens sometimes worry that eye strain has damaged their retina. Digital eye strain more commonly relates to focusing effort, reduced blinking, dry eyes or an uncorrected prescription. However, a detailed examination remains worthwhile if vision is persistently blurred, headaches are new or symptoms do not settle with appropriate visual and dry-eye support.

What Happens If an Image Shows Something Unusual?

An unexpected image does not automatically mean there is a serious problem. Retinal features vary between individuals, and imaging can reveal harmless findings that simply need recording. Your optometrist should explain what has been seen in clear language, whether it needs routine monitoring and what the next steps are.

If there is a concern requiring specialist assessment, an appropriate referral can be arranged. Bringing previous records into the picture, where available, can be very helpful because stability over time is often as clinically meaningful as the image itself.

Do not wait for a routine appointment if you develop sudden loss of vision, a curtain or shadow in your sight, a sudden increase in flashes or floaters, severe eye pain, or a new distortion in central vision. These symptoms need urgent assessment.
